Job Description

- Coordinate care transitions across inpatient, outpatient, and home settings.
- Conduct patient and family consultations to support care planning.
- Facilitate discharge planning and follow-up care.
- Collaborate with primary care physicians and interdisciplinary teams.
- Monitor patient progress and promote self-management.
- Document care activities and maintain compliance with protocols.
- Active and unrestricted RN license in Florida.
- Bachelor’s degree in nursing (Associate degree with four or more years of experience accepted).
- Minimum of 2 years clinical experience in a healthcare setting.
- Valid FL driver’s license and reliable transportation.
- Strong communication, organizational, and decision-making skills.
- Proficiency in electronic health records and HIPAA compliance.
- Experience in care management, utilization review, or discharge planning.
- HMO experience preferred.
- CCM certification is a plus.
Remote Case Manager
Job Description
This role involves supporting patients as they transition between different care settings, ensuring continuity and quality of care. The nurse will work collaboratively with care teams, patients, and their families to coordinate resources, manage care plans, and optimize recovery using clinical expertise and motivational coaching.

Work Environment
This is a remote position, but candidates must reside in Treasure Coast, Tampa, Citrus, Northeast Florida, or Orlando.
Pay and Benefits
The pay range for this position is $40.00 - $45.00/hr.
