      About the Role

      We are seeking a detail-oriented, highly skilled Clinical Coordinator / Patient Triage Specialist to join our team in a fully remote capacity. In this role, you will serve as a key point of contact for patient care, managing clinical triage, patient communication, and vital administrative workflows. The ideal candidate is a Registered Nurse (RN) or experienced clinical professional fluent in both English and Spanish, capable of operating independently in a fast-paced virtual healthcare environment.

      Key Responsibilities
      • Patient Triage & Clinical Assessment:
        • Conduct virtual patient triage, evaluating symptoms and clinical concerns.
        • Accurately document clinical information and history in the Electronic Health Record (EHR).
        • Escalating urgent or critical clinical issues to attending providers in a timely manner.
      • Administrative & Patient Care Coordination:
        • Manage patient messages, call backs, and general clinical inquiries via patient portal and phone.
        • Coordinate specialist referrals, prior authorizations, and prescription refills.
        • Handle patient scheduling and manage provider appointment calendars.
        • Perform general virtual office support tasks to ensure seamless daily clinic operations.
      • Patient Communication:
        • Provide clear, empathetic, and professional communication to patients in both English and Spanish.

      Qualifications & Requirements
      • Licensure / Experience: Registered Nurse (RN) preferred; experienced clinical professionals (e.g., LPN, Medical Assistant with extensive triage experience) will also be considered.
      • Language Proficiency: Fully bilingual in English and Spanish (written and spoken) is required.
      • Clinical Skills: Strong background in patient triage, clinical documentation, and risk escalation protocols.
      • Software Experience: Experience using eClinicalWorks ( eCW ) is strongly preferred.
      • Specialty Background: Prior experience in Rheumatology is highly preferred.
      • Remote Competency: Proven ability to work independently, manage time effectively, and maintain professionalism in a virtual work setting.
      • Soft Skills: Exceptional interpersonal and communication skills, high emotional intelligence, and attention to detail.
