      What to Expect (Job Responsibilities):
      - Strengthen the connection between patients and their primary care physicians/medical practice staff
      - Assist patients in navigating their care and addressing health concerns
      - Document patients' medical histories and issues in the electronic health record (EHR)
      - Build trusting relationships with medical practices and the Guidehealth organization
      - Utilize effective communication methods to engage and schedule high-risk patients

      What is Required (Qualifications):
      - Certified or Registered Medical Assistant with 2 or more years of experience
      - Must be bilingual (English/Spanish)
      - Strong interpersonal skills with a focus on empathy and professionalism
      - Proficiency in Microsoft Office and experience with electronic health records
      - Reliable transportation and a valid state driver's license for occasional travel to provider sites

      How to Stand Out (Preferred Qualifications):
      - Experience with motivational interviewing and behavioral change management
      - Familiarity with social determinants of health and their impact on patient care
      - Ability to navigate multiple technology platforms and software effectively
