      What to Expect (Job Responsibilities):
      - Engage with patients to navigate their healthcare and improve communication with medical practice staff
      - Collaborate with clinical teams to manage a targeted population of patients
      - Document patients' medical histories and issues in the electronic health record (EHR)
      - Assist patients in implementing care plans and closing care gaps, while escalating medical issues as needed
      - Utilize various communication modalities to engage high-risk patients and schedule appointments

      What is Required (Qualifications):
      - Certified or Registered Medical Assistant with a minimum of 2 years of experience
      - Bilingual in English and Spanish
      - Strong interpersonal skills with a focus on empathy and professionalism
      - Proficiency in Microsoft Office and familiarity with various electronic health records
      - Reliable transportation and a valid state driver's license for occasional travel to provider sites
