      What to Expect (Job Responsibilities):
      - Engage patients to improve their health outcomes and navigate their care
      - Collaborate with medical practice staff and case management teams to manage patient populations
      - Document patients' medical histories and issues accurately in the electronic health record (EHR)
      - Assist in implementing high-impact care plans and addressing care gaps
      - Communicate effectively with patients through various modalities to schedule high-risk patients

      What is Required (Qualifications):
      - Certified or Registered Medical Assistant or Certified EMT with 2 or more years of experience
      - Strong interpersonal skills with a focus on empathy and professionalism
      - Proficiency in documenting patient interactions in real-time using technology
      - Knowledge of medical terminology and the function of primary care practices
      - Ability to work independently in a remote environment

      How to Stand Out (Preferred Qualifications):
      - Familiarity with various electronic health records such as eClinicalWorks, EPIC, Athena, or Cerner
      - Experience with motivational interviewing and behavioral change management
      - Competency in navigating multiple technology platforms and tools
      - Reliable transportation and a valid state driver's license for occasional travel
