      What to Expect (Job Responsibilities):
      - Strengthen connections between patients and their primary care physicians/medical practice staff
      - Engage patients to navigate their care and improve communication with medical practices
      - Document patients' medical history and issues accurately in the electronic health record (EHR)
      - Manage a targeted population of patients in collaboration with medical practice staff and case management teams
      - Implement assigned Key Performance Indicators to enhance patient health outcomes and reduce medical spending

      What is Required (Qualifications):
      - Certified or Registered Medical Assistant or Certified EMT with 2 or more years of experience
      - Strong interpersonal skills with a focus on empathy and professionalism in all interactions
      - Experience and understanding of medical terminology and the primary care physician practice
      - Proficiency in Microsoft Office, including Outlook, Word, and Excel
      - Ability to work independently in a remote environment and maintain patient confidentiality

      How to Stand Out (Preferred Qualifications):
      - Familiarity with various electronic health records such as eClinicalWorks, EPIC, Athena, or Cerner
      - Experience with motivational interviewing and behavioral change management
      - Exceptional conversation skills and ability to document patient interactions in real-time
