What to Expect (Job Responsibilities):
- Assist patients in arranging transportation to and from the hospital for ongoing medical treatment
- Collaborate with unit-based case managers to facilitate discharge plans to other psychiatric or medical settings
- Research and maintain information on community social resources
- Educate patients and families about available resources and how to access them
- Plan and implement presentations regarding community resources
What is Required (Qualifications):
- High School Diploma or Equivalent required; Associate's Degree in a related field preferred
- 2-3 years of related experience
- Familiarity with medical terminology and community services/resources
- Strong interpersonal skills and ability to maintain effective working relationships with patients and families
- Excellent written and verbal communication skills
How to Stand Out (Preferred Qualifications):
- Experience in crisis intervention and assessment
- Strong collaborative skills in a complex, fast-paced environment
- Knowledge of current community resources
