      RN Registered Nurse Case Manager - Fresno, CA

      Service area: Fresno and Madera counties

      Schedule; Monday through Friday

      Qualifications


      On An Average Day You Will:
      (includes, but not limited to)

      Make a true difference—one patient, one family, one compassionate moment at a time.

      • Develop and implement individualized care plans based on current and prior medical history

      • Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding

      • Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey

      • Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure

      • Administer physician-ordered medications and treatments with skill and empathy

      • Educate patients and their families on care techniques and symptom management

      • Track and communicate changes in patient condition, ensuring timely and effective responses

      • Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided

      • Collaborate with interdisciplinary teams to ensure comprehensive care delivery

      • And contribute to other supportive duties as part of a compassionate, mission-driven team

      • And additional duties as assigned


      What You'll Bring to The Team:

      • A heart for hospice care and a commitment to exceptional service

      • A valid Registered Nurse license in good standing in the state you will work in

      • A valid driver’s license and reliable transportation

      • CPR certification (current)

      • Strong observational and communication skills—both written and verbal

      • A flexible, team-oriented attitude and willingness to support additional duties as needed
