What to Expect (Job Responsibilities):
- Initiate outreach through various channels, including phone, SMS, email, and social media, to engage qualified individuals for Enhanced Care Management
- Deliver clear explanations of ECM benefits and guide participants through the onboarding process
- Build rapport with clients using motivational interviewing and cultural humility to address their unique needs
- Collaborate with team members to enhance outreach efforts and improve referral pipelines
- Document interactions accurately and provide performance summaries to supervisors
What is Required (Qualifications):
- Strong communication skills in verbal, written, and digital formats
- Resilient and goal-oriented mindset, able to handle rejection effectively
- At least 3-5 years of experience in sales, outreach, or cold calling, or 2-3 years in a healthcare setting
- Proficiency with CRMs, HIPAA-compliant chat platforms, and telehealth tools
- Ability to adapt communication style for diverse populations
How to Stand Out (Preferred Qualifications):
- Knowledge of Medi-Cal/Medicare systems
- Multilingual fluency
- Experience working with underserved or high-need populations
