What to Expect (Job Responsibilities):
- Conduct in-home assessments on patients, including medication reviews and health screenings
- Provide patient education by aligning care desired with the best care available
- Identify diagnoses for assessment in care management and medical management
- Partner with the Regional Medical Officer and other team members to develop appropriate care plans
- Coordinate care with multiple stakeholders, including PCPs, specialists, and ancillary providers
What is Required (Qualifications):
- Active Nurse Practitioner or Physician Assistant license
- Master's degree from an accredited NP Program or PA program
- Ability to communicate positively, professionally, and effectively with others
- Effective problem-solving, organizational, and time management skills
- Active RN License and Furnishing number (for NP only), NPI Number, DEA, Valid BLS, and a valid Driver License with current automobile insurance
How to Stand Out (Preferred Qualifications):
- One (1) year of prior clinical or home care experience
- Previous EMR experience
- Experience in the care of older adult (geriatric) patients
