      Overview

      Are you a compassionate individual ready to kickstart your career in addiction treatment? BrightView Health is seeking proactive and empathetic individuals to join our team as Targeted Case Manager I.

      Targeted Case Manager I is an entry-level case management position in the specialty of Substance Use Disorder (SUD), focused on building confidence in the provisioning of case management services for

      BrightView’s patients.

      Responsibilities

      PATIENT ASSESSMENT AND PLANNING:

      • Assess patient eligibility for specific service needs.
      • Develop individualized integrated action plans for each patient.
      • Evaluate effectiveness of recommended services and adjust recommendations as needed.

      RESOURCE REFERRAL AND MONITORING:

      • Refer and connect patients and families with available resources.
      • Monitor adherence to recommendations and follow up accordingly.
      • Assist patients in accessing appropriate medical, social, education, and employment resources.

      DOCUMENTATION AND REPORTING:

      • Complete detailed reports on needs assessment and case management activities.
      • Ensure timely and comprehensive input of all necessary case management data into the electronic patient health record.
      • Ensure timely and accurate completion of documentation, meeting assessment and treatment plan completion expectation deadlines.

      PATIENT SUPPORT AND COORDINATION:

      • Provide crisis and de-escalation techniques.
      • Ensure patients have reliable, consistent transportation to BrightView and other care providers.
      • Coordinate patient care with other BrightView team members.

      PROFESSIONAL DEVELOPMENT AND COMPLIANCE:

      • Attend required meetings, including individual and group supervision, treatment team meetings, and quarterly in-service.
      • Adhere to all necessary regulatory and company requirements in documentation and evaluation.
      • Participate in TCM audit, meeting assessment and treatment plan completion expectation deadlines, and ensuring appropriate grammar and use of technical/medical terminology.

      KNOWLEDGE SKILLS, AND ABILITIES

      • Excellent verbal and written communication skills
      • Highly empathetic and compassionate to effectively support the recovery journey of BrightView’s patients
      • Competent at working with a diverse population of colleagues and patients
      • Natural problem solver, looks for solutions to best meet patient needs with a sense of urgency
      • Team-player, able to work collaboratively in a multidisciplinary healthcare environment
      • Adaptable and agile within a dynamic work environment
      • Technologically capable, comfortable operating in multiple systems for communication and documentation purposes. Familiarity with MS Office software (Outlook, Teams, Word, etc) preferred
      • Embraces BrightView’s culture of compliance - operates with a high degree of integrity and compliance to work standards and regulatory requirements
      • Familiarity with HIPAA and 42-CFR Part 2 desired
      • Prior experience with harm reduction a plus

      Qualifications

      EXPERIENCE

      Required: 1 year of experience working with adults in a human service setting.Preferred: Prior experience with harm reduction.EDUCATION:Required: Bachelors Degree in a related Behavioral Science field. BENEFITS AND PERKS:
