      What to Expect (Job Responsibilities):
      - Provide clerical and administrative support for Care Management in the Population Health Division
      - Receive and upload documents from provider practices into the care management platform
      - Assist with data collection and the preparation of medical records for multifunctional use
      - Coordinate with providers, stakeholders, and the public to provide necessary information and assistance
      - Support nursing assignments and assist nurses with their duties as needed

      What is Required (Qualifications):
      - High school diploma or GED required; Associate degree in Health or Human Services preferred
      - Minimum of six (6) months of experience in a healthcare field
      - Certified Medical Assistant (AAMA) certification required
      - Proficient knowledge of healthcare and medical terminology
      - Ability to maintain confidentiality and establish effective working relationships with various stakeholders

      How to Stand Out (Preferred Qualifications):
      - Experience with community-based programs and services related to physical health, mental health, and substance use
      - Strong communication skills, both oral and written
      - Skilled in the use of personal computers and related software applications
