What to Expect (Job Responsibilities):
- Conduct home orientations and assist with signing of necessary paperwork
- Perform initial assessments of new patients during orientations
- Conduct periodic supervisory visits and check-ins for ongoing care cases
- Train caregivers and patients as required per specific cases
- Collaborate in interdisciplinary meetings with care managers, caregivers, and patients
What is Required (Qualifications):
- Minimum of 2 years of experience working with frail or elderly disabled adults and caregivers
- Valid driver's license and insured vehicle for travel within the state
- Excellent verbal and written communication, organizational, and time management skills
- Proficient in Microsoft Office Suite (Word, Excel, Outlook)
- Strong critical thinking and problem-solving skills
How to Stand Out (Preferred Qualifications):
- Previous field experience in a medical or case management environment
- Experience with data input and case review/case note entry; familiarity with Salesforce is a plus
- Hands-on experience in home care
- Working knowledge of ADL/IADL assessments
- Excellent training skills and a team-player mentality
