      The Health & Wellness Coach provides telephonic individual health coaching to promote lifelong, sustainable change toward optimal wellness. The program takes a holistic approach, and provides support in areas such as weight management, fitness, nutrition, tobacco & nicotine cessation, diabetes prevention, sleep management, stress management, as well as other common health & wellness topics.

      Responsibilities:

      • Provide personalized telephonic coaching using proven strategies and evidence-based practices to empower and facilitate individuals in making behavior and lifestyle changes
      • Determine client’s baseline wellness level through assessment techniques
      • Understand client’s desired level of wellness & further elicit motivation to work toward achieving goals
      • Collaborate with clients to develop a client-centered plan
      • Ongoing assessment of client’s stage of change, barriers, motivation and progress
      • Manage personal caseload, complete all scheduled appointments on time and properly document coaching session information within case management system
      • Participation in training and monthly team meetings as scheduled
      • Project work and administrative tasks as needed

      Qualifications:

      • Bachelor's degree in a health and wellness related field (psychology, nutrition, exercise science, health promotion, nursing).
      • Health & Well-being Coach Certification preferred
      • National Board-Certified Health and Wellness Coach Certification (NBHWC) preferred
      • Experience with tobacco & nicotine cessation
      • Bi-lingual French required
      • 2+ years’ experience with health and wellness coaching or related field
      • Must be able to demonstrate solid coaching skills including, but not limited to: active listening, appreciative inquiry, goal setting, motivational interviewing
      • Strong organizational and time management skills with experience working independently in a self-directed environment
      • Excellent professional communication skills to provide utmost customer service, as well as effective communication with coworkers
      • Experience with problem solving and working effectively with others, using good judgment in new and challenging situations
      • Working knowledge of chronic and comorbid health conditions related to wellness, and their impact on behavior change
      • Capacity to learn and adhere to a wide range of protocols; strong attention to detail and follow through on assigned tasks
      • Ability to work virtually with access to reliable high-speed internet service, computer, phone, headset and workspace for equipment; must be able to conduct private, confidential conversations, in compliance with HIPAA regulations.       • Proficient computer skills, including working knowledge of Microsoft Office applications.
