- Plan and implement programs specifically designed to address the population/subpopulation needs.
- Evaluate, design, present, recommend, and disseminate culturally appropriate health education information and materials.
- Develop and present health education and promotion programs, such as training workshops, conferences, and school or community presentations.
- Develop content for posting on various digital media outlets, including ensuring consistency across communication channels and working within state requirements and guidelines.
- Develop and maintain cooperative working relationships with agencies and organizations interested in public health care.
- Maintain databases, mailing lists, contact networks, and other information to facilitate the functioning of the health education programs.
- Ensure communication activities are based on principles, methods, and best practices. Use data and analyses to develop optimal communication strategies and messages, and to evaluate their impact.
- Observe confidentiality of health records in accordance with MPC policies and procedures.
- Work from home position. Must be available and willing to travel up 50% of the time to meet with members, for community-based outreach events, and for in-person team meetings.
- Knowledge of concepts and processes for providing health education and communication services. This includes needs assessment, meeting quality standards, and evaluation of satisfaction and learning.
- Knowledge of human behavior and performance; unique differences in ability, personality, and interests, learning and motivation.
- Valid Driver's License.
- Attention to detail and accuracy.
- Experience with Microsoft Office Suite, with proven expertise in Microsoft Excel.
- Bachelor's degree in public health, health communication and education, and/or health-related field; master's degree preferred.
- 3+ years' recent work-related experience in public health setting or equivalent of combination of education and experience.
- Some statistical training/experience preferred.

Summary/Position Objectives:
This position helps to develop and implement health promotion and disease prevention programs as part of the Population Health program at Maryland Physicians Care. This position is responsible for providing and managing health education programs that help individuals, families, and their communities maximize and maintain healthy lifestyles. The incumbent is responsible for using data to identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage healthy lifestyles, policies, and environments to include linking health systems, health care providers, and members to address individual and population health needs. May also serve as a resource to assist individuals, other health care professionals, or the community and may administer fiscal resources for health education programs.
