      What to Expect (Job Responsibilities):
      - Engage with patients to improve communication with their primary care physician and navigate their healthcare needs
      - Collaborate with medical practice staff and case management teams to manage a targeted patient population
      - Document patients' medical histories and issues accurately in electronic health records (EHR)
      - Assist in implementing care plans, addressing patient concerns, and escalating medical issues as needed
      - Maintain professional relationships with medical practices and meet assigned Key Performance Indicators

      What is Required (Qualifications):
      - Certified or Registered Medical Assistant OR Certified EMT with 2+ years of experience
      - Proficiency in motivational interviewing, critical listening, and behavioral change management
      - Strong interpersonal skills with a focus on empathy and professionalism
      - Experience with medical terminology and primary care physician practices
      - Ability to work independently in a remote environment and navigate multiple technology platforms

      How to Stand Out (Preferred Qualifications):
      - Familiarity with electronic health records such as eClinicalWorks, EPIC, Athena, or Cerner
      - Exceptional written, visual, and verbal communication skills
      - Knowledge of social determinants of health
      - Experience in patient coordination for individuals with complex medical and social needs
