      What to Expect (Job Responsibilities):
      - Engage with patients to navigate their healthcare needs and improve communication with medical practice staff
      - Collaborate with clinical teams and manage a targeted population of patients to achieve better health outcomes
      - Document patients' medical history and issues accurately in the electronic health record (EHR)
      - Assist patients with referrals and connect them to community-based organizations as needed
      - Meet assigned Key Performance Indicators (KPIs) to enhance patient health outcomes and reduce medical spending

      What is Required (Qualifications):
      - Certified or Registered Medical Assistant OR Certified EMT with 2 or more years of experience
      - Strong interpersonal skills, with a focus on empathy and professionalism in all interactions
      - Proficiency in Microsoft Office applications, including Outlook, Word, and Excel
      - Ability to work independently as part of a patient's overall care team under the supervision of licensed professionals
      - Reliable transportation and a valid state driver's license for occasional local travel

      How to Stand Out (Preferred Qualifications):
      - Experience with electronic health records such as eClinicalWorks, EPIC, Athena, or Cerner
      - Familiarity with motivational interviewing and behavioral change management techniques
      - Knowledge of social determinants of health and their impact on patient care
      - Exceptional written, visual, and verbal communication skills
