      What to Expect (Job Responsibilities):
      - Engage patients and assist them in navigating their healthcare, addressing questions and concerns
      - Collaborate with medical practice staff and case management teams to manage a targeted population of patients
      - Document patients' medical history and issues accurately in the electronic health record (EHR)
      - Implement and meet assigned Key Performance Indicators to enhance patient health outcomes
      - Escalate medical issues appropriately to clinical staff and maintain a trusting relationship with medical practices

      What is Required (Qualifications):
      - Certified or Registered Medical Assistant or Certified EMT with 2 or more years of experience
      - Strong interpersonal skills with a focus on empathy and professionalism
      - Proficiency in Microsoft Office and familiarity with various electronic health records
      - Ability to navigate technology and manage multiple tasks independently
      - Reliable transportation and a valid state driver's license for occasional travel

      How to Stand Out (Preferred Qualifications):
      - Experience with motivational interviewing and behavioral change management
      - Knowledge of social determinants of health
      - Exceptional written and verbal communication skills
