What to Expect (Job Responsibilities):
- Build trusted relationships that empower patients to take control of their health
- Engage with patients through calls, texts, and emails to connect them with appropriate care
- Partner with physicians, nurses, and care teams to coordinate care and improve outcomes
- Accurately capture each interaction in the electronic health record (EHR) for seamless communication
- Travel occasionally (10-15%) for in-person engagements and conducting retinal eye exams
What is Required (Qualifications):
- Certified or Registered Medical Assistant or Certified EMT with 2+ years of experience
- Exceptional empathy, communication, and listening skills
- Ability to thrive in a remote environment while collaborating with cross-functional clinical teams
- Experience navigating EHR systems (eClinicalWorks, Epic, Athena, or Cerner) and Microsoft Office
- Strong attention to detail and ability to manage multiple priorities
