What to Expect (Job Responsibilities):
- Engage with members in the community (60-80%) and by phone (20-40%), providing support and encouragement
- Conduct comprehensive assessments covering clinical, psychosocial, and behavioral health needs
- Utilize motivational interviewing and active listening to help members identify goals and develop personalized care plans
- Connect members to resources addressing social determinants of health and assist in overcoming barriers to care
- Document interactions, assessments, and referrals, while staying updated on technology changes
What is Required (Qualifications):
- Peer Specialist Certification, lived experience with SUD/OUD/Mental Illness, or 1-2 years of experience as a Peer Support Specialist
- High school diploma or GED
- Strong knowledge of SUD/OUD member management and experience in community or clinical settings
- Excellent communication and organizational skills, with the ability to multi-task in a fast-paced environment
- Valid driver's license and auto insurance; ability to travel for face-to-face visits and navigate Philadelphia's public transit
