- To provide and deliver an advanced nursing service for people with specialist needs in both the hospital and out-patient setting.
- To provide anticoagulation and thrombosis advice and support to patients, their families and other healthcare professionals following diagnosis and through treatment.
- Work within the Nurse-Led VTE clinic: Interpreting ultrasound scans, completing a clinical assessment, including examination and history taking - training will be provided.
- Initiating treatment after full assessment of risk and contra indications to treatment
- Ordering diagnostic tests and interpret the results.
- Work within the anticoagulation clinic, GP and consultant referrals, bridging clinic etc.
- To accept responsibility for patient referrals, ensuring all patients have an accurate plan of care, and all paperwork is completed.
- To ensure continuity of a high standard of evidenced-based care through identifying and initiating appropriate steps for effective care.
- Provide a warfarin dosing service on behalf of our local GP surgeries with support from the anticoagulation clinical software 4S DAWN

- Experience of autonomous working
- Cardiology experience
- Demonstrates competence in clinical skills relevant to the clinical setting
- Excellent communication skills, including written and spoken communication
- Good IT skills and knowledge of MS Office
- Ability to do a presentation using presentation software packages.
- Ability to problem solve, anticipate and forward plan

Job summary
An exciting opportunity has arisen for a Band 6 nurse to join the Salisbury Anticoagulation and Thrombosis Team.
The role involves seeing patients in both the in-patient and out-patient clinical areas assessing, treating and advising patients, carers and allied health care professionals on various aspects of anticoagulation and thrombosis care.
The ideal candidate should already be working as a Band 6 or Band 5. If a Band 5, they must have at least two years worth of Band 5 experience when applying.
The successful candidate needs to be enthusiastic and motivated. They should have good communication and time-management skills They must be able to work well autonomously, within a team and collaborate with members of the multi-disciplinary team. The ideal candidate should also have good IT skills as the use of a computer and several different IT platforms is essential for this role.
Anticoagulation & Thrombosis experience is not necessary as full training will be provided. Candidate will also be required to undertake training in assessment and history taking and will be expected to become an independent prescriber.
The post is 30 hours over 4 days.
Our working hours are Monday to Friday, 9:30am to 5:30pm.
