      Job summary

      An exciting opportunity has arisen for a dynamic, motivated and innovative Assistant Practitioner to join the nursing team on Ward 7 Prince Philip Hospital.

      Ward 7 specialises in Breast, Colorectal, General and Urology Elective Surgery.

      We are seeking a forward thinking, professional practitioner with excellent communication, interpersonal, and organisational skills to provide gold standard evidenced based care to our patients.

      The successful candidate can look forward to working in a proactive and supportive professional environment.

      A structured competency framework and training will be provided. The focus of learning will be professional skill specific to the role, and completion of a competency portfolio.

      Main duties of the job

      Undertake identified duties in relation to the care environment and the direct care of the patient(s)/client(s) as identified and delegated by a registered nurse in accordance with the Health Board policies and procedures.

      Perform duties and activities within the framework of a plan of care, as delegated and reviewed by a registered nurse at regular intervals.

      Assist patients/clients to achieve physical and emotional well-being and comfort.

      Ensure that any changes in patients/client's condition or circumstance are reported immediately to the appropriate person/registered nurse/nurse in charge.

      Utilise transferable skills within level of competence.

      NB: General and transferable clinical care competencies, and area-specific competencies required for this role are detailed in the accompanying 'Competency Booklet' (Appendix 1).

      Understand and interpret the findings of the initial assessment of the patient's care requirements (as completed by a registered nurse).

      Contribute to the reassessment of health and well-being needs of patients/clients, and the associated documentation, within scope of competence.

      Respect people's dignity, wishes and beliefs, involve them in shared decision making and obtains consent for delegated procedures within scope of competence.

      Record and report back accurately and fully on the reassessments undertaken and risks identified. Participate in developing actions in the care plan to address identified risks.

      • Certificate in a relevant health related subject i.e. 120 credits at academic level 4 (or equivalent award/experience) OR has held previous NMC registration as a registered nurse
      • GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications
      • Evidence of continuing personal development relevant to the role in health care
      • Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines and the all Wales Band 4 Assistant Practitioner Governance Framework.
      • Clinical knowledge which is relevant to the role.
      • Demonstrates understanding of the importance of following procedures and treatment plans
      • Demonstrates an understanding of the legislation related to Safeguarding e.g. adults/children, The Mental Capacity Act and Deprivation of Liberties Safeguards to keep adults within their care safe

      Desirable

      • Local knowledge of statutory and voluntary services
      • Teaching or assessment qualification
