      Job summary

      An exciting opportunity has arisen for 2 Band 6 Primary Care Practitioner to join our Primary Care Mental Health Team working within our Primary Care Networks (PCNs) in west Birmingham.

      Within BSMHFT our community mental health services are well-established and as partof the BSOL community transformation programme, we are looking to increase our primary care teams with primary care practitioner roles. This is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care. The development of our Primary Care mental health team is an essential service which we are in the process of evolving in line with the NHS Long Term Plan.

      We are looking for enthusiastic and dynamic individuals who are able to work within a multi- disciplinary team, working alongside GP's and other primary care professionals who strive to achieve the best outcomes for individuals within the local community.

      Main duties of the job

      You will be responsiblefor providing mental health triage, assessment and outcomes based intervention to meet service users needs within the Primary care mental health team.

      You will be responsible for identifying need and best course of advice, support or intervention, promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.

      Crucial to this role will be your ability toensure robust communication across mental health providers, VCSE organisations and primary care in accordance with each individual service users needs by provide support and advice to all colleagues with the PCN as required including participation within the local neighbourhood MDT's.
