What to Expect (Job Responsibilities):
- Oversee and manage four teams, ensuring operational excellence and team morale
- Lead performance improvement efforts to enhance workflow effectiveness and quality
- Develop and maintain reporting mechanisms for tracking and monitoring outcomes
- Ensure compliance with federal, state, and agency policies and standards
- Conduct audits and quality checks to uphold service standards
What is Required (Qualifications):
- Registered nurse with an active license in Tennessee or a Master's in Social Work with an active unrestricted license
- Minimum of 5 years of supervisory experience
- At least 3 years of experience in care coordination for long-term care or home and community-based services
- Proficient in Microsoft Office (Outlook, Word, Excel, and PowerPoint)
- Valid driver's license and willingness to travel up to 90% of the time

- Experience with the CHOICES program or similar care coordination initiatives
- Strong analytical and decision-making skills
- Excellent oral and written communication skills
- Proven ability to build relationships and work collaboratively across departments
