      Job summary

      The Care Navigator plays a crucial role in providing support and guidance to individuals navigating the healthcare system. This position involves coordinating care, managing patient information, and ensuring that patients receive the necessary resources and assistance. The ideal candidate will possess strong organisational skills, motivation, excellent phone etiquette, and a background in administrative tasks.

      Main duties of the job

      Duties

      • Assist patients in understanding their healthcare options and available services.
      • Coordinate appointments and follow-ups with healthcare providers.
      • Maintain accurate records of patient interactions and care plans using data entry skills.
      • Utilise Microsoft Office and SystmOne for documentation and communication purposes.
      • Manage administration tasks such as repeat prescriptions, new registrations and general enquiries. This list is not exhaustive.
      • Provide administrative support to the healthcare team as needed.
      • Demonstrate professionalism in all communications, both over the phone and in written correspondence.
      • Strict Confidentiality is required within this role.
      • We require flexibility on days & hours, which are to be agreed, but they will all require to be working up to 6.30pm.
