- Performs independent visits while operating under standing order procedures.
- Partners with provider on chronic care management and complex patient management.
- Performs medical flip visits with provider for hospital discharges, ER follow-ups, and other complicated visits.
- Facilitates general day-to-day flow of the clinic in close collaboration with other team members.
- Serves as liaison between outside providers/health care entities and the clinic.
- Collaborates with appropriate teams to design and test workflows and RN integration into the care team.
- Performs patient, family, and caregiver education including motivational interviewing and patient centered health action plans.
- Assists in improving the performance of identified quality measures and population health initiatives.
- Triages patients in clinic and over the phone as requested.
- For staff assigned to a specialty support role:
- Responsible for case management related to specific patient populations to ensure utilization of specialized healthcare services.
- Conducts regular assessment of patient needs and develops comprehensive plan of care.
- Collaborates with patient and Care Team for wrap around services.
- Conduct home health visits as determined in the care plan.
- Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.

Base Salary Range: $38.03 - $57.52 USD hourly
Salary is determined based on applicable experience and education. External offers typically do not exceed the mid-point of the pay range.

Essential Duties and Responsibilities:
Qualifications:
Education/Experience: Associate degree in nursing required. Valid RN license in the state of Washington or Idaho required upon hire. Nursing experience, preferably within a community health setting required. Case management experience preferred. Experience with electronic health records preferred.
Skills: Excellent customer service, oral and written communications skills required. Compassion for patients required. Must be able to work well in team environment. Ability to multi-task in fast-paced work environment, with ability to anticipate future needs required. Computer skills required. BLS (CPR/AED) required. Valid driver’s license and insurance required. Commitment to supporting a safe and respectful environment is required.
Physical Demands:
Nurses are required to stand, sit, and be mobile one-third to two-thirds of the time. They are required to use hands to finger, handle or feel over two-thirds of the time, as well as reach with hands and arms. Climbing or balancing occurs less than one-third of the time; while stooping, kneeling or crouching occurs between one third and two-thirds of the day. Communicating by talking / hearing occurs over two-thirds of the day. Amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time up to 25 pounds. Rarely is there a need to lift more than 26 pounds.
