- Conduct inbound and outbound calls to complete 30-day patient satisfaction surveys
- Assess members' physical and psychosocial status
- Follow up on certified health conditions, current care plans, and program services
- Document all interactions and findings in the internal database
- Pay: $39-$41 per hour (based on experience)
- Schedule: Per Diem, remote position (hours may vary based on business needs)
- Active RN license in New Jersey
- Minimum of 1 year of RN experience in the United States
- Current CPR certification
- Home care experience preferred

Care Finders Total Care, a Modivcare Home Care company, is seeking a Registered Nurse (RN) to manage a caseload of members and conduct 30-day patient satisfaction and quality assurance (QA) surveys via telephone. This role focuses on assessing members' physical and psychosocial needs while maintaining accurate nursing documentation in our internal systems.
