      Job summary

      An exciting opportunity has arisen within the Enhanced Team East.

      We are looking to recruit an enthusiastic Band 6 Community Psychiatric Nurse to cover maternity leave who is committed to delivering a high quality service and who can work within a team approach.

      You will be responsible for supporting with the assessment process for patients managing a caseload, as well as providing information and support for those accessing the service. You will be supported by an experienced team of practitioners in an integrated multi-disciplinary approach.

      This position is available for 37.5 hours per week at Baghill House & Wellbeing Centre, Pontefract. The team covers the East of Wakefield district. Access to a motor vehicle is necessary. Our work is carried out Monday to Friday; 8:30am to 5.00pm. Weekends and bank holidays 9:00am to 5:00pm.

      While this role meets the eligibility requirements for Skilled Worker sponsorship, due to the short-term nature of the contract, the Trust will not be offering sponsorship for this post.

      Main duties of the job

      Care Coordination of a caseload of service users with complex mental health needs.

      Working with both internal organisations to improve outcomes, manage risk and deliver effective care

      Care planning

      Risk Management

      Supporting service users to reach optimal levels of mental wellbeing , engage in local communities and achieve personal targets.

      Work with a small creative and dynamic team who offer excellent support to each other.

      Deliver mental health interventions and treatments in the community

      Requires a resilient, passionate and empathetic team player who can think outside the box to encourage engagement of service users that present with complex mental health needs.

      Main duties of the job

      Contribute to excellent care for our service users.

      Develop and lead on care planning risk assessment and formulation

      Deliver treatment interventions in the community

      Work closely with our inpatients unit to support safe discharge

      Initial assessment of service users being referred to the service

      Supporting depot treatments both in community and withing depot clinics

      Working closely with partner organisation

      Referring to and taking part in safeguarding processes

      Supporting less senior members of staff and students in supervisory roles.

      Keeping clear and accurate records.

      Supporting compliance with our clinical governance framework.

      5. Ensures safe carriage, storage and administration of drugs prescribed (registered nurses only).

      8. Keeps the Management Team and colleagues informed of developments relevant to the area of responsibility.

      Education :

      1. Supervises students and trainees on placement.
