Job Description

- Team of 19 Primary Care Physicians, 12 Advanced Practice Providers with a 1:1 MA ratio
- Strong internal referral network
- Physician input in clinical decisions, scheduling, and staffing
- Epic EMR
- Onsite Care Navigators, Nurse Allies, Social Workers, and Behavioral Health Providers
- Lab, Imaging and Immediate Care available onsite.
- Flexible Monday - Friday Schedule with opportunity for a 4-day work week
- Choice of outpatient only or hybrid inaptient/outpatient
- Base compensation: $280,000 – $350,000
- Total target: $250,000 – $700,000+ with incentives
- Sign-on bonus, relocation assistance, loan forgiveness, CME allowance
- Paid licensure and certification renewals
- Up to 5 weeks time off and paid holidays
