Healthcare Support Worker - Band 3 - Darwin Centre:

There is an exciting opportunity to work as a Healthcare Support Worker within the Darwin Centre. We are searching for extra special Healthcare Support Workers who have experience, passion and pride when caring for younger persons and this vacancy would suit someone who is looking for an exciting opportunity and wants to work on a Young Persons Ward.

Main duties of the job
To provide support, and spend therapeutic time with an allocated group of children and young people. To promote and support the physical health, comfort, wellbeing and recovery of children and young people, and to assist them in re-establishing their place in the community, as appropriate. To assist Registered Professionals in assessments and care planning/ implementation and evaluation of individualised care plans. To provide support and day-to-day supervision to junior staff members as required, demonstrating the Health Care Support Worker role and role modelling good practice.

1. Communicates with children and young people and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.
2. Regularly reports and effectively communicates children and young people health status and care aims in care records and verbal reports to Registered Health Professional staff members and other members of the multi-disciplinary team.
3. Relates appropriately with children and young people, carers, colleagues and others at all times maintaining professional boundaries.
4. At all times ensures that own actions, support the equality, diversity, rights and responsibilities of individuals.
5. Identifies, minimizes and reports risks of danger, harm and abuse to senior colleagues.
6. At all times minimizes risks to self and others by undertaking safe working practices.
7. At all times works within agreed and established Trust policy and operational procedures.
8. Works with other Trust services to promote seamless service-user person centered care from referral/admission, to transfer and discharge on identified care pathways.
9. Responsible for contribution towards planning and undertaking delivery of planned therapeutic activities with an identified group of service users.
10. Responsible for undertaking elements of a holistic assessment, under the direction of Registered Health Professional staff. Responsible for regularly working with children and young people to negotiate and review existing plans to address identified needs under the supervision of Registered Professionals.
