What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews, and physical evaluations
- Build trusted relationships with patients to enhance their healthcare journey
- Utilize technology to streamline clinical workflows and documentation during evaluations
- Provide diagnostic and preventive services as needed, such as spirometry and bone density screening
- Engage with patients to answer health-related questions and guide them towards appropriate care
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to travel locally and within licensed states as needed
- Experience in conducting health evaluations or similar clinical responsibilities
- Flexibility to adapt to various patient needs and environments

- Experience in home health or community-based healthcare settings
- Familiarity with digital health tools and technologies
- Strong interpersonal skills and a compassionate approach to patient care
