What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews, and physical assessments
- Engage with patients to answer health questions and provide educational guidance
- Utilize technology, such as an iPad, to facilitate clinical workflows during evaluations
- Travel locally within licensed states as needed, conducting virtual visits when appropriate
- Deliver diagnostic and preventive services as indicated by health plans
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification for Nurse Practitioners or Physician Assistants
- Ability to perform duties comfortably in diverse home environments
- Flexibility to travel locally and conduct virtual visits as required
How to Stand Out (Preferred Qualifications):
- Multi-state licenses for Nurse Practitioners or Physician Assistants
- Experience in conducting home health evaluations or similar roles
