      Job summary

      Pharmacists are an essential part of the current and future NHS workforce and as such an exciting opportunity has arisen for an experienced Clinical Pharmacist to join our supportive and forward thinking Practice.

      As Lead Pharmacist you will offer clinical leadership to the existing Pharmacists and mentorship to the Pharmacy team and Pharmacy Technicians. You will be instrumental in tailoring the design and delivery of pharmacy interventions and policies that address Practice and Bassetlaw wider health and care priorities and ensuring that the medicines management processes within the practice are optimised to achieve effective, safe, and patient centred prescribing.You will hold an independent prescribing qualification and will undertake patient consultations as appropriate with areas of responsibility that include chronic disease management, patients with complex polypharmacy and patients with multiple long-term conditions.

      Main duties of the job

      The main duties of the role are:

      • To lead and develop the Pharmacy team
      • To ensure safe, effective, and efficient medicines management processes are in place
      • To undertake a patient facing role and undertaking clinical medication reviews to proactively manage patients with complex polypharmacy
      • To provide pharmacy advice and guidance
