      Job summary

      The post holder is responsible for the delivery of nursing care to the practice population. They will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions especially in Diabetes, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

      Main duties of the job

    • Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being
      • Implement and evaluate individual treatment plans and management plans for patients with a known long-term condition
      • Perform appropriate test for the diagnosing and reviewing of patients with Long Term Conditions (Diabetes)
      • Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition
      • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
      • Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care
      • Deliver opportunistic health promotion using opportunities as identified with individual patients.
      • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions

      • Deliver opportunistic health promotion using opportunities as identified with individual patients.

      • Liaise with other health professionals and multidisciplinary teams in the best interest of the patient.
      • Utilise own skills and knowledge to deliver safe, evidence-based practice, adhere to local and National policies and Nice Guidelines.
      • Implement and participate in vaccination and immunisation programmes for adults, such as Flu vaccines, pneumococcal and shingles vaccines to patient accessing the surgery.
      • Undertake community visits for house bound patients as identified individually and within own scope of practice.
      • Restocking and maintaining consulting and clinical rooms if stock supplies are low, as appropriate.
