      As the Choice Exceptional Needs Care Coordinator (ENCC) you will evaluate, investigate, determine and review extended care coordination needs for clients with complex needs including clients with specialized mental health care requirements, in order to promote the availability and quality of individualized community based services and supports, primarily housing. In this position you will conduct clinical consultation and assessments to facilitate service coordination for individuals/adults with serious mental health problems, to limit institutionalized care and yet live in the least restrictive setting possible. You will evaluate and determine the best specialty services and available resources with community providers, to meet the client's treatment plan.

      Teleworking: At this time, this position is designated to “routine telework,” meaning you will be working remotely (from home) most of the time; a hybrid schedule also is available. As with all routine telework positions, employees may be required to come into the office on occasion. The designation of Routine Telework may be subject to change at a future time. Currently, all employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations.

      The eligible list created from this recruitment may be used to fill regular, full or part time, temporary, and limited duration assignments.

      As the Mental Health Consultant, Choice Exceptional Needs Care Coordinator , other duties will include:

      Case Coordination

      • Provide intensive care coordination, case coordination, and evaluations for clients with complex needs including clients with specialized mental health care needs to ensure that client needs are appropriately and adequately addressed by service providers.

      • Client specialized health-care addressed, may include complex medical needs, high health care needs, housing needs, special mental and physical healthcare needs and co-morbidities, multiple chronic conditions, behavioral health issues including chemical dependency, and those identified as requiring care coordination and specialized resources.

      • External partners may be discharge planners from state hospitals, residential treatment providers, case managers, substance use disorder treatment providers, acute care settings, community corrections, police, and other mental health providers. Initiate, attend, and participate in regularly scheduled and urgent/ impromptu staffings to collaboratively problem solve any barriers to services/discharge planning.

      • Locate and assist with arranging specialty mental health services, as appropriate, upon request by client, physician or provider, to ensure that client needs are appropriately and adequately addressed.

      • Exchange risk assessments for clients with physicians or providers to increase the knowledge of overall risk of specific clients who have been identified with a mental health diagnosis to determine if specialty service is appropriate under the client's treatment plan.

      • Review authorization patterns for capacity and length of stay for housing or other needs.

      • Provide clinical recommendations to community partners regarding needs and recommendations for services needed.

      Documentation

      • Document all coordination efforts in the electronic health record (Evolv NX), including, but not limited to, communication with service/placement providers, member/provider contacts and meeting attendance/staffing.

      • Complete assessments based on clinical information to provide clinical input on level of need/placement, and gaps in care.

      • Use wording and statements that can be understood and are appropriate for the diverse coordinated partners; client files/records may be accessed to support the client.

      Team Events

      • Share knowledge and updates learned as part of standard work and training attended so updates are known by team members to best support client needs.

      • Participate in learning and Subject Matter Expert groups as needed to advance and update policies and procedures to ensure bias is removed from processes to gain best outcomes for a diverse client base.

      Minimum Qualifications/Transferable Skills*:

      Education/Certification: Must meet the standards of Qualified Mental Health Professional - C (QMHP-C) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at MHACBO website , QMHP-C and/or licensed Board of Oregon (LPC, LCSW, PhD, etc.) (Required) AND

      • Graduate degree in psychology or

      • Graduate degree in social work or counseling or

      • Graduate degree in a behavioral science field

      Preferred Qualifications/ Transferable Skills*: You do not need to have the following preferred qualifications/ transferable skills to qualify. However, keep in mind we may consider some or all of the following when identifying the most qualified candidates.       • Appropriate Oregon Licensing Board LCSW, LCP, Clinical Psychologist

      • Leadership experience in the crisis field

      • Leads with race through actions and advocacy with internal programs, with community partners and across departments throughout the county.

      • Demonstrate the ability to flex style when faced with myriad dimensions of culture in order to be effective across cultural context.

      • Respect and appreciation for ethnic and cultural diversity.

      • Knowledge of challenges and barriers faced by those with Severe and Persistent Mental Illness (SPMI) and the systems they encounter in Residential Services, Adult Protective Services and Crisis Services.

      • Ability to collaborate and build relationships to achieve positive work outcomes.

      • Ability to maintain a safe and healthful workplace.

      • High degree of resilience, is outcome driven and can thrive in an environment of rapid change while effectively managing pressure in an effective and professional manner.
