  Care Navigator Registered Nurse RN, Orthopedics, Full-Time, Days
  

  We are looking for a Care Navigator Registered Nurse (RN) to join our Orthopedic team in Albuquerque, NM.

  
  This job is located at Lovelace Medical Group, a multispecialty provider group, which includes clinics, doctors and advanced practice providers in a broad range of specialties and primary care, and provides patients with exceptional health care through the support of Lovelace Health System hospitals and technology.

  
  Responsibilities
  
  

  Care Navigator Registered Nurse RN Job Responsibilities:

  • This position is the primary liaison for the continuity of care and social support systems for patients and their families throughout their health care experience.
  • Develops education and training programs for patients, physicians and care staff on the appropriate clinical, community requirements and programs.
  • Guides cancer patients through their course of clinical care working with a variety of medical practitioners, facilities, and community resources.
  • Works with community advocates for the patient.
  • Tracks and trends data for the program reporting requirements.
  • Adheres to policies, procedures and regulations to ensure patient safety and compliance.

  
  Qualifications
  
  

  Care Navigator Registered Nurse RN Education and Experience Requirements:

  • Minimum: Bachelor in Nursing or equivalent required.
  • Master’s in health related field preferred
  • 1-2 years Navigator experience preferred,
  • Excellent communication skills
  • Ability to communicate effectively;
  • Ability to work independently;
  • Basic computer skills.
