Job Description
- Req#: 85129
- This position is the primary liaison for the continuity of care and social support systems for patients and their families throughout their health care experience.
- Develops education and training programs for patients, physicians and care staff on the appropriate clinical, community requirements and programs.
- Guides cancer patients through their course of clinical care working with a variety of medical practitioners, facilities, and community resources.
- Works with community advocates for the patient.
- Tracks and trends data for the program reporting requirements.
- Adheres to policies, procedures and regulations to ensure patient safety and compliance.
- Minimum: Bachelor in Nursing or equivalent required.
- Master’s in health related field preferred
- 1-2 years Navigator experience preferred,
- Excellent communication skills
- Ability to communicate effectively;
- Ability to work independently;
- Basic computer skills.
