- Registered Pharmacist in states of assigned pharmacies
- Outstanding communication skills
- Excellent time management and organizational skills.
- Ability to respond positively and quickly to rapidly changing priorities
- Extensive knowledge of applicable state laws, patient care guidelines, common disease states and therapies encountered in the community pharmacy setting
- Successful completion of company approved training programs including immunization, diabetes training, patient care pharmacist onboarding
- Experience with Outcomes MTM
- CPR certified
- Strong understanding of pharmacy operations
- At least two years of active involvement in patient care services or completion of post-graduate clinical residency
- Established collaborative relationships with other healthcare professionals in the community
- Strong presentation skills
- Successful completion of formal training programs provided by ACPE accredited CE providers in the areas of MTM, diabetes and cardiovascular care.
- Local and long-distance travel may be required
- Incumbent will travel among various pharmacy locations
- Incumbent may be exposed to bodily fluids and blood borne pathogens
- Incumbent may be exposed to cleaning preparations to comply with OSHA sanitation requirements
- Incumbent may be exposed to medical sharps and other medical wastes
- Standing for long periods of time may be required in completion of job duties
- Stooping, bending, twisting and reaching may be required in completion of job duties
- Provides & trains pharmacy staff to provide patient care services to meet the needs of patients and the local community including, but not limited to, medication therapy management (MTM), chronic condition management, immunizations, medication administration, and biometric screenings
- Successfully identifies & communicates CMR & TIP opportunities for all assigned pharmacy locations to assist with execution of the service at store level. Set expectations & follows up with assigned pharmacy teams to ensure CMR & TIP completion.
- Provides ongoing training and support to ensure assigned pharmacy teams execute against their MTM opportunities.
- Documents all services according to company policy and payer specifications
- Ensures confidentiality of all patient records
- Coordinates coverage for time dedicated to patient care services, working with the Division Pharmacy Manager (DPM), Patient Care Service Manager, and/or local scheduler
- Communicates (in a timely manner) & provides support to District Pharmacy Manager with any operational deficiencies seen in assigned patient care sites.
- Accepts assignment of patient care sites, which are subject to change
- Creates and manages schedule following established procedures to allow for central scheduling of patient appointments in all assigned patient care sites
- Responsible for coordinating Medication Administration trainings and meeting patient needs within assigned stores with the Specialty Care Team in Boise.
- Maintain 3 months of rolling availability in, Schedule360, the on-line clinical scheduler.
- Travels among assigned patient care sites on a regular basis, providing appointment-based services, trainings (including medication administration), educating pharmacy teams about patient care initiatives
- Marketing services to patients and store customers and visiting healthcare providers, local businesses and community groups
- Performs patient care services telephonically when needed
- Attends and participates in approved scheduled meetings and trainings with divisional or corporate patient care service leaders via conference call, in person or via other media
- Reports expenses following company policy
- Serves as a preceptor for student pharmacists, summer interns and residents where applicable
- Participates in health fairs and other community-based programs
- Completes and shows proof of specified continuing education in targeted disease states as required by clients, payers and/or Albertson/Safeway
- Performs various presentations for groups of patients, healthcare professionals, pharmacists, students and interns
- Patient Care Pharmacists must be flexible to work weekend, evening and holiday schedules to meet patient and business needs

Pay Transparency
The pay range is $72 to $77 per hour, but no less than the local minimum wage. Starting rates will vary based on things like location, experience, qualifications. Dependent on length of service, hours worked benefits include medical, dental, vision, disability and life insurance, sick pay (accrued based on hours worked), PTO/Vacation pay (accrued based on hours worked), paid holidays (between 7-9 days annually), bereavement pay, and retirement benefits (pension and/or 401k eligibility).”
