      Overview

      Fairview Health Services is looking for a Patient Placement Specialist to help us in coordinating inpatient admissions and transferring patients to ensure a smooth process flow for the patient, our staff and our hospitals.

      This exciting role helps make a difference by working behind the scenes to ensure the patients can help get the care they need.

      The Patient Placement Specialist will enter demographic patient information into the electronic health record, as well as patient transfers and discharges in a timely and accurate manner. They will facilitate communication, admission and consultations for physicians while answering phone lines dedicated to physicians accessing M Health Fairview services. They will assist all callers with the highest quality of customer service while facilitating multiple calls simultaneously.

      About this position:

      Hours: This position will work 48 hours, every two weeks (0.6 FTE) and is benefit eligible

      Shifts: Evening

      Weekend rotation: Every Third weekend

      Location: Onsite at the Fairview Midway Campus, 1700 University Avenue in Saint Paul

      Starting at: $20 per hour

      Responsibilities

      Responsibilities

      The Patient Placement Specialist works in a call center environment. During this time they will:

      • Receive incoming admissions through the admission line
      • Answer the direct emergency admission line for M Health Fairview, collect and enter any needed information, be organized and prioritize while using critical thinking skills for best outcome.
      • Coordinate emergency inpatient admissions to acute care units from external sources.
      • Coordinate same day inpatient admissions to acute care units from internal and external sources.
      • Coordinate internal and external physician-to-physician phone consultations.
      • Conference with the RN Patient Placement, the accepting physician, and the sending physician to collect appropriate data, and under the direction of the Patient Placement Manager, appropriately places the patient on the correct floor according to clinical service or acuity.
      • Communicates admissions to patient care units and to referring site.
      • Coordinate all transferring patients and discharges
      • Facilitate inpatient consultations

      Qualifications

      Qualifications

      Minimum Qualifications:

      • 1-2 years of customer service experience

      Preferred Qualifications:

      • Healthcare experience or related degree
      • Patient placement experience in a call center environment
