Hazeldene Medical Centre for a Referral Coordinator to join our team and complete various administrative tasks to provide the best quality care services to the patients. We are looking for a person who is passionate about developing their career in the healthcare sector, understands the value of being sensitive and maintaining confidentiality, and possesses effective communication and organisational skill .
Main Responsibilities:
1. Referral Management: Receive, review, and process referrals from healthcare providers, including primary care physicians, specialists, and other healthcare professionals.
2. Patient Assessment: Evaluate patient referrals to determine the appropriate level of care or specialty required. Gather and analyse relevant medical information, including medical records, test results, and other necessary documentation. Provide appropriate clinical information to specialist
3. Communication: Serve as the primary point of contact for patients, healthcare providers, and specialists throughout the referral process. Facilitate effective communication and information exchange between all parties involved. Referral coordinators act as a liaison between the primary care physician, specialist, and patient. They provide updates to all parties involved, ensuring that everyone is informed about the referral status and any additional requirements.
4. Communicate effectively with team members, patients and carers and outside agencies to enhance the teams performance and meet patients needs.
5. Documentation and Record-keeping: Maintain accurate and up-to-date records of patient referrals, appointments, and related communication. Enter referral data into electronic health records (EHR) or referral management systems as appropriate.
6. Follow-up and Tracking: Monitor the progress of referrals and ensure timely completion of appointments. Follow up with patients and healthcare providers to gather feedback, address concerns, and provide updates on referral status.
7. Referral Guidelines and Policies: Stay updated on referral guidelines, protocols, and policies established by the General Practice.
8. Quality Improvement: Participate in quality improvement initiatives aimed at optimising the referral process, improving patient satisfaction, and enhancing overall coordination of care.
9. Training and Education: Provide education and guidance to healthcare providers, staff, and patients regarding referral processes, requirements, and available services.
10. Participation in all mandatory training sessions and attending any training programme implemented by the practice and taking responsibility for own development and learning.
