Responsibilities
Job Summary:
The Social Worker in Neurology offers services to patients and families in the clinics listed below. As a member of the interdisciplinary team, the social worker assists patients and families through advocacy in negotiating the hospital and community systems (working with community agencies), concrete services (i.e. PGW/PECO, releases of information related to psychosocial issues, problems with insurance creating a barrier to access to health care, Medical Assistance letters, counseling (referrals to Mental Health agencies and Psychiatric issues and emergencies), and support. For all new patients, a social work assessment will be completed when there is a social work need. Social Work can assist, as a member of the team, with insurance appeals process and can also assist/facilitate with pre-authorizations, continued pre-authorizations and/or certifications related to DME or related requests, as appropriate, and with other requests as deemed appropriate by the Social Worker. The social worker also acts as a liaison to community agencies and referral sources. The social worker provides consultation and training to the professional community and participates in Practice Intake coverage system, as required.
Qualifications
Training & Education:
Masters Degree from an accredited school of Social Work.
Skills & Abilities:
Knowledge of developmental stages of children and adults. This position interacts with patients from all ages from Neonates to Adults. Knowledge of basic computer skills is an asset.
Experience:
Minimum 5 years’ graduate experience under MSW supervision.
