      About our Pharmacy Service

      You will have responsibility for assisting the department in providing a comprehensive clinical pharmacy service to all patients at Colchester Hospital.

      You will have the opportunity to work across all clinical specialities, including providing support to the Oncology and Haematology pharmacy team.

      You will work closely with departmental and Trust colleagues to ensure the provision of an integrated and efficient clinical pharmacy service.

      Clinical activity will be split between the Collingwood Centre/West Bergholt Ward and other clinical areas (it is expected that oncology/haematology activity and other rotational activity will form an approximate 50:50 split). You will be expected to work predominantly at ward/departmental level and be an integral part of the multidisciplinary team.

      If not already being worked towards, it is expected that you will undertake, or further progress through the Postgraduate Diploma in General Pharmacy Practice.

      Support will also be provided to ensure you achieve any additional training deemed necessary regarding working with Haematology and Oncology patients throughout the Trust.

      Main duties of the job

      Undertake the Medicines Reconciliation process for admitted patients in line with agreed Trust and departmental guidance.

      Ensure, with clinical pharmacy colleagues, that pharmacy-led, level-2 Medicines Reconciliation activity is prioritised as a key clinical pharmacy activity and that prioritised patients also have a level-3 clinical pharmacyreview.

      In conjunction with the departmental SAM lead and senior pharmacy colleagues:

      Proactively encourage the utilisation of the Trust SAM procedures across the organisation.

      Assist nursing colleagues in identifying those patients suitable to self-administer their own medicines
