      GENERAL DESCRIPTION

      Performs highly advanced and/or supervisory (senior-level) social service or case management work that involves locating community resources and/or long term alternative placements for youth with special needs and/or circumstances who are committed to the agency, in preparation for their upcoming release from a TJJD program. Facilitates the placement process or resource accrual with TJJD facility personnel and outside agencies. Such placements include but are not limited to placement in a long term community setting specifically to address the youth’s potential homelessness, intellectual and/or physical disability, and/or the youth’s mental health issues. Locates new potential placement opportunities for TJJD and forwards this information to TJJD Executive Administration. Responsible for agency collaboration with community resource personnel, local and state mental health authorities, local and state rehabilitative service agencies, local and state child placement agencies, parole officers, and private residential providers. Works under minimal supervision with extensive latitude to use initiative and independent judgment. This position reports to the Assessment and Placement Program Administrator and may be responsible for the Centralized Placement Unit (CPU) in the supervisor’s absence. May be required to travel or work overtime.

      ESSENTIAL FUNCTIONS

      Serves as the primary agency liaison between TJJD and the Texas Department of Family and Protective Services (DFPS). Tracks and monitors all TJJD/DFPS crossover youth and assists in direct case management services and long term permanency placement planning for these youth. Initiates and facilitates meetings between agency personnel and DFPS. Participates in ongoing local and state collaborative workgroups to improve policy, procedures, and overall service provision internally and externally for youth receiving dual services.

      Participates in interagency committees and work groups which are involved in legislative change and policy creation and revision to provide continuity of care for youth as they transition through various state systems. The focus of these committees includes juvenile justice, adolescent mental health, and permanency planning for at risk youth.

      Performs advanced case management services for youth transitioning to alternative placements upon reentry into the community. Facilitates, attends and participates in meetings to ensure appropriate and timely short- and long- term placement as well as resource provision for youth with special needs/circumstances is secured prior to youth release.

      Coordinates the collection of information and materials required for determining long-term placement options available to youth in need of placement beyond a TJJD facility. Researches available community resources available for these youth.

      Establishes and maintains professional relationships with local and state mental health service agencies, local and state child care placement agencies, residential treatment facilities, and local and state rehabilitative service agencies. Acts as a liaison between these providers and the agency.

      Provides consultative services and technical assistance to case managers, psychologists, psychiatrists, and other field staff regarding placements in alternate settings to avoid indefinite placement within TJJD’s correctional environment.

      Initiates and facilitates youth specific staffings with internal and external stakeholders as a method for obtaining community resources for the youth upon release.

      Facilitates meetings among agency personnel to discuss case specific strategies for youth with intensive mental health or medical needs.

      Serves as an intra and inter-agency committee member for various work groups.

      Provides coverage as senior placement specialist when needed.

      Performs a variety of related duties not listed, to be determined and assigned as needed.

      Performs all duties in compliance with agency safety policies and procedures.

      MINIMUM QUALIFICATIONS

      Bachelor’s degree. Four years of full-time wage earning experience that may be any combination of the following: coordinating services for at-risk or disadvantaged populations, social work, case management services, or experience in an adult or juvenile correctional, mental health, or detention setting in the placement of youth, or related area. Completed course hours of graduate study may be substituted for up to two years of experience on a basis of 12 graduate course hours for one year of experience.

      OR

      High School Diploma or GED. Eight years of full-time wage earning experience that may be any combination of the following: coordinating services for at-risk or disadvantaged populations, social work, case management services, or experience in an adult or juvenile correctional, mental health, or detention setting in the placement of youth, or related area. Completed course hours from an accredited college or university may be substituted for up to four years of the required years of experience on a basis of 30 undergraduate course hours for one year of experience.

      Preferred:

      - Bachelor’s degree with major course work in social sciences, behavioral science, criminal justice (not law enforcement or police services), or related field.

      - One year of experience as a project lead or in a supervisory/management role.

      Note: Any degree(s), semester hours, diploma(s), or GED required to meet minimum qualifications must be obtained from an accredited educational institution.
