What to Expect (Job Responsibilities):
- Engage, educate, and promote member decisions related to achieving and maintaining optimal health status through assessment and planning
- Conduct standardized health risk appraisals (HRAs) and relevant assessments, verifying medical history and current health needs
- Meet daily production member outreach standards
- Utilize web-based software to document assessments, activities, and education provided to members
- Develop individualized care plans based on member needs and preferences
What is Required (Qualifications):
- At least 1 year of experience in a healthcare setting working with underserved or special needs populations, or equivalent education and experience
- In some states, a current Community Health Worker (CHW) certification may be required
- In some states, an associate’s degree in a healthcare-related field may be required
- Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job-related travel
- Strong customer service skills and excellent written/verbal communication skills

- Proficiency in Microsoft tools (Word, Excel, and ideally OneNote)
- Ability to work independently and collaboratively within a team
- Experience working effectively in a remote environment
