      Job summary

      We are looking for an Associate Practitioner to join our Community Nursing Team on the Isle of Wight.

      Do you have excellent interpersonal skills and a passion for learning and development in the field of therapies aimed at avoiding hospital admission? You will need to demonstrate flexibility, reliability and excellent organisational skills. Highly motivated, you will enjoy working as part of a dynamic and developing integrated team providing holistic, person centred care across a variety of community settings.

      The successful applicant will be the first point of contact for the service, providing assessment and prescription of programs, spanning Physiotherapy, Occupational therapy and Nursing. This can include optimising mobility, re-training personal and domestic activities of daily living, building confidence and providing appropriate multi-disciplinary input and guidance.

      In return for your skills and commitment we offer regular supervision and support from qualified health care professionals, training tailored to your role and learning needs. You will benefit from working with experienced and supportive colleagues from a variety of clinical backgrounds and be able to develop your skills in working with adults requiring therapy services.

      Main duties of the job

      To work as part of the interdisciplinary team to provide comprehensive physical therapy to avoid hospital admission.

      Assist in identifying health and social needs of patients and participate in the delivery and evaluation of care provided under the indirect supervision of qualified staff (Registered Nurses, Physiotherapists and Occupational Therapists).

      Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals.

      Qualifications

      Essential

      • NVQ Level 3 or BTec National in health and social care.
      • Knowledge of nursing and/or physiotherapy / occupational therapy / speech and language therapy procedures and practices.
      • Working towards or willingness to work towards a level 5 AHP Support Worker Apprenticeship.

      Desirable

      • Formal Assistant Practitioner qualification/ Health and Social Degree or equivalent.
      • Nursing Associate Qualification registered with the NMC.
      • Foundation Degree in Health and Social Care Practice.

      Experience

      Essential

      • Knowledge of nursing and / or physiotherapy / occupational therapy / speech and language therapy procedures and practices.
      • Significant experience of working in a relevant health or social care setting.
      • IT skills.
      • Demonstrate effective communication skills, written and verbal.

      Desirable

      • Experience of working within Physical Rehab.
      • Experience of working in the community.
      • Knowledge of Rehabilitation
      • Broad knowledge of community services
