      As care coordinator you will play a central role in ensuring safe high quality person centred care across our services you will work closely with operational and quality teams offering clinical leadership and supporting best practice in line with cqc standards

      Experience

      Essential

      • minimum 3 years post registration experience in a clinical or Care Coordination role
      • experience in supported living residential care or community adult health

      Desirable

      • Strong understanding of CQC, DoLS, MCA, and risk management frameworks.
      • Excellent communication, leadership, and organisational skills.
      • Ability to travel between services (mileage reimbursed).

      • Experience with forensic clients or dual diagnosis
      • Experience supporting services through cqc inspections

      • NMC Registered Nurse (RMN, RNLD, or RGN with community/complex needs experience).
