      The overall purpose of case management in the Onslow County Detention Center is to assess an offender’s criminogenic needs and connect them to treatment and services, helping them remain substance and crime free once released into the community. The Case Manager will work within the Onslow County Detention Center and conduct assessments for all assigned non-violent adults with substance use or co-occurring mental illness and substance use. High priority will be given to those with moderate to high risk of reoffending to reduce the risk of criminal justice recidivism. The Case Manager will coordinate with the current correctional behavioral health providers along with the medical unit and therapy staff to prepare individualized case plans, including referrals to treatment and services upon release from the Onslow County Detention Center. In addition to recovery-based services, the Case Manager will address housing, employment, education and health and wellbeing. Case management services and support takes place both in the Onslow County Detention Center and in the greater Onslow County community and surrounding counties if indicated. Case management will continue for a minimum of 90 days after release with 6 month and beyond data tracking.

      This grant position is grant funded by the Comprehensive Opioid, Stimulant, and Substance Use Program and Opioid Settlement Funds. The continuation of the position will be determined by the availability of funding.

      Case Management : Essential Functi on, 60%
      • Complete assessments to determine criminogenic and social determinant of health needs
      • Complete an intake that includes orienting the client to the program and service expectations
      • Collaborate with both the client and the START team to include medical, therapy and detention center staff to formulate a case plan to address the identified needs for community reentry.
      • Make appropriate referrals to address the identified post-release needs (referrals may include but are not limited to job training, mental health/substance use treatment, health care, public assistance, vocational rehabilitation, education, employment, housing, peer support, and other community-based services).
      • Obtain Release of Information from client to facilitate communication with service providers.
      • Monitor the client’s progress and re-evaluate interventions regularly and when necessary, at a minimum of every 30 days after case plan creation.
      • Follow up with released offenders and referral sources to monitor the linkage as well as compliance to services.
      • Complete discharge summaries to reflect the client’s progress and compliance to case plans after release.
      • Document all interactions with client and service providers in the client’s case record daily.

Examples include written communications that clearly outline the situation and action items, verbal presentations, and briefings to the Program Manager and members of the Management Team.

This includes being available and responsive to issues and concerns as they arise.
      Knowledge, Skills & Abilities
      • Knowledge of the criminal justice system is preferred.
      • Must be able to keep strict confidentiality and work with diverse populations.
      • Must have knowledge of available resources in the service area for adults with serious mental illness and or substance use
      • Must have knowledge of and comply with state and federal statutes, rules and policies that effect the target population
      • Must be able to coordinate individualized, integrated mental health, substance abuse and co-occurring services through case management to meet the needs of individuals served
      • Must be able to use a computer and Microsoft Office programs.
      • Must possess great oral and written communication skills
      • Must be able to work within the guidelines established in the policies and procedures of Onslow County Detention Center and Onslow County Government
      • Must be able to work cooperatively and collaboratively with multidisciplinary staff
      • Must be able to recognize signs and symptoms of mental illness and substance use
      • Must be able to effectively relate to a variety of individuals, including persons living with a mental illness and/or substance use disorder and co-workers
      • Must be able to problem solve and advocate for persons served
      • Must be able to uphold individuals' rights and adhere to HIPAA guidelines
      Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and a minimum of one (1) year of full time or equivalent experience working with adults experiencing serious mental illness and substance use along with one (1) year of case management experience.

      • Valid North Carolina Driver License required by first date of employment with good driving record.
