What to Expect (Job Responsibilities):
- Serve as a point of contact on Hospital/ED calls to assist with discharge planning and coordination of care needs
- Provide short-term care coordination on a case-by-case basis, working to expedite safe and timely discharges
- Link and refer members, guardians, and hospital staff to various services or community resources
- Participate in cross-functional projects or tasks to enhance service delivery
- Review and maintain data, information, and documentation relevant to service coordination
What is Required (Qualifications):
- High School Diploma/GED with six (6) years of experience in behavioral health, developmental disabilities, or substance use; or
- Associate’s degree with four (4) years of experience; or
- Bachelor’s degree with a minimum of two (2) years of experience; or
- Equivalent combination of education and experience
- Must have a valid driver’s license and reside within North Carolina
- Must be able to travel within the catchment area as required
How to Stand Out (Preferred Qualifications):
- Experience providing crisis intervention or crisis response to individuals within the behavioral health, developmental disabilities, or substance abuse population
- Fluent Spanish speaker
