- Use holistic patient assessment skills of adults and children so that clinical decision making will ensure prompt and appropriate action in the best interest of the patients/families well being.
- Undertake assessment of their health and presenting complaint, diagnosing, treating, giving advice and discharging or referring patients to other health professionals including the GP and speciality on call teams according to protocols.
- Request and interpret X-rays and blood tests if clinically appropriate.
- Act as an effective role model, through the application of skills to patient care and acting as advisor to the wider MDT.
- Work under pressure in a rapidly changing environment.
- Prescribe medications under own professional scope of practice and within the formulary acknowledging own professional boundaries.
- Keep accurate and immediate records of every enquiry including assessment and advise regarding patient care using both CRS and Adastra
- Encourage patients to carry out self care as appropriate through effective communication and health care advice.
- Be responsible and accountable for all the patients on own caseload, seeking guidance from ACP/GP when clinically appropriate.
- Maintain own professional development. Identifies any training needs and attend relevant courses necessary to ensure best practice.
- Keep up to date with current developments in patient care and evidence based practice.
- Work within all policies, protocols and guidelines of the UTC and THGPCG.
- Ensure that confidentiality is maintained at all times including the storage of records and the transfer of verbal and written information.
- Work within multidisciplinary care teams promoting good communication with the Trust and hospital staff, statutory and voluntary agencies.
- Ensure that patients and carers who face language barriers are helped to understand and make use of the service available. Take into account cultural belief and values of the patient.
- Identify any safeguarding issues patients may have and act on the information.
- Contribute to the recruitment and selection process.
- Support other members of the health care team.
- Ensure new staff are familiar with and adhere to THGPCG policies and clinical pathways and policies.
- Take part in the day to day management of the team at the UTC.
- Maintain effective communication with stakeholders and other outside agencies promoting a positive image of the UTC.
- Contribute to the development, monitoring, auditing and reviewing of policies, procedures protocols and guidelines within the UTC.

- Previous experience in an Emergency Care/Acute setting.
- NMC or HCP Registration with no cautions or conditions of practice.
- Advanced Physical Assessment & Minor Injury Assessment qualification at Level 7.
- Independent non-medical prescribing qualification.
- Substantial post registration experience.

Main duties of the job
The post holder is an experienced autonomous clinician who, acting within their professional boundaries, will work flexibly and collaboratively with other members of the UTC team to provide care for the presenting patient. This will include initial history taking, clinical assessment, forming differential diagnosis, interpreting investigations where required, independent prescribing and planning treatment with the patient which may include self-care or referral to speciality teams for both adult and paediatric illness and injury patients.
The role will initially be face to face within the UTC however there is also opportunity to provide safe clinical decision making and expert care over the telephone working from home taking NHS 111 Direct booking calls for UTC streamed patients. Prospective candidates will be expected to rotate into a variety of shift patterns that include weekends and night duties as the service is 24-hours.
For interested clinicians there would be opportunities to progress to becoming and Advanced Clinical Practitioner following the 4 pillars of advanced practice.
