- Conducts a family needs assessment and aligns with the family to develop a care plan or an IRP (Individual Rehabilitation Plan).
- Uses a motivational interviewing approach with the youth and family as they pursue goals.
- Coordinates meetings and communicates both verbally and in writing with outside providers, especially schools.
- Uses knowledge of local family supports and helps the family to access these supports which can include supports for basic needs, medical and transportation.
- Keeps a written record of work, in particular the family assessment and ongoing meetings with family members or providers. This will include documentation in the electronic medical record such as contact notes and treatment plans.
- Provides updates of family progress as needed including quarterly reporting.
- Perform other duties as assigned.
- High School/GED and 5 years of work experience with emotionally disturbed children and adolescents and their families.
- Bachelor’s degree in social work, Psychology, or related field and one year of work experience with emotionally disturbed children and adolescents and their families
- Shows reliability by being punctual to meetings with family members and outside agencies.
- Excellent relational skills with courteous demeanor toward families and providers.
- Can develop a schedule in collaboration with their supervisor to meet the needs of an assigned caseload.
- Ability to remain calm in stressful situations.
- Ability to engage families who are experiencing setbacks and discouragement.
- Ability to work with small groups of children or family members.
- Excellent communication skills including active listening.
- Service-oriented and able to work independently.
- Requires the ability to utilize computer systems and software necessary to perform position functions. Basic Windows PC, web browsing (i.e., Chrome, Internet Explorer, etc.), and Microsoft Outlook skills required. Other Microsoft Office application knowledge, such as Word, Excel PowerPoint, Skype, and OneDrive desired. FCC will be expected to use the Electronic Medical Record for documentation daily.

- Hearing and vision abilities within normal range (corrected) to drive safely and to observe and communicate with others.
- Driving (Agency and/or personal vehicle) is required for this position. Driver must be at least 21 years of age, with a minimum of 2 years of driving experience, and a valid driver’s license issued by the state of residence. There can be no state-issued restrictions on the licenses that would impede a driver’s ability to operate the vehicle as required by the Agency, and there must not be more than three (3) points on their driving record.

Salary: $52,500
Catholic Charities of Baltimore, Villa Maria Behavioral Health is currently seeking Family Care Coordinator who will provide trauma-informed with a child and their family when there is a significant impairment in the child’s functioning in one or more of the domains of school, family, and/or community. Grant funding is available to support this position. The FCC uses a family engagement tool to identify strengths and needs of the family and eventually creates a care plan with goals the family helps to create. The FCC supports the family’s pursuit of these goals using their knowledge of the community and support agencies as well as motivational interviewing skills with the ultimate goal of improving child and family functioning in the domains identified. The work schedule will vary between Monday-Friday 11AM - 7PM. Schedule is flexible and hybrid between in office, home visitations and remote work.
Villa Maria Behavioral Health Clinics provide mental and behavioral health services to children, adults, and families across Maryland. These services include individual counseling, family counseling, psychiatric evaluation, social and coping skill development, addiction counseling and treatment, and medication management.
