      Job summary

      An exciting opportunity has arisen for a Senior Nurse to join The Rosemary Foundation Hospice at Home charity as our clinical lead.

      We are a Nurse Led service providing palliative care over a 24 hour period and responding to SOS calls. You will be responsible for leading Registered Nurses and Health Care Assistants to provide high quality, safe and responsive nursing care that is evidenced based.

      You will be an integral member of the Senior Leadership Team and you will support the Registered Manager in ensuring the delivery of excellence in palliative care nursing.

      As a charity we have autonomy in how we manage our services, we are not time restricted in care delivery, enabling us to spend the time patients and their families need at the end of their lives and supporting them to die in their preferred place of care.

      We are a cohesive and supportive team, as a charity the clinical team is involved in fund raising activities, where we engage with families and the public, have lots of fun and create positive memories for all.       Main duties of the job

      To lead the day to day coordination of the clinical service and team

      To lead the team to manage complex patient care needs with compassion and efficiency

      The post holder will have a strong clinical presence in assessing and evaluating individual patients needs and identifying appropriate interventions for patients and their families

      To ensure staff are appropriately trained, using a variety of mediums to deliver training and develop staff

      The post holder will be skilled in collaborative working with a variety of Health Care Professionals , we often have a 'shared care' approach with our key partners, so ability to work collaboratively is important

      To ensure appropriate Governance and Quality Assurance processes are in place

      To support our volunteers , ensuring volunteers have easy and direct access to clinical support whilst undertaking their visits to patients homes

      Excellent communication and interpersonal skills alongside a dynamic and motivated attitude are key requirements for this role

      The post holder is a key member of the Senior Leadership Team- Registered Manager, Finance Manager, Operational & Fundraising Manager & Clinical Lead and is responsible for providing clinical expertise, leadership and development to Nurses and HCAs, to ensure delivery of excellence in palliative care services to patients and their families in the community.

      Key Accountabilities, Responsibilities &Tasks

      Role Specifics

      • To Lead and Manage the Hospice at Home Clinical Team

      • Take part in the TRF On Call Rota.

      • To support and direct assessment of the care needs of patients and their families referred to TRF.

      • To carry out and / or supervise the carrying out of specific treatments and clinical procedures. To base clinical practice on current evidence-based practice.

      • To oversee the management of effective triage and assessment of new referrals within the Team.

      • Provide leadership to the clinical team engaging with key partners and the wider Specialist Palliative Care MDT Frameworks.

      • Participate and attend Governance and End of Life meetings in support of strong effective relationships with stakeholders.

      • Work effectively with Multi-Disciplinary Teams across care boundaries supporting patients choice to be at home in a safe environment.

      • To be responsible for the clinical team, ensuring effective retention and recruitment of staff and support sickness and well-being management.

      • To maintain continuing personal contact with patients and evaluate the effectiveness of care delivered.

      • To assess the needs of patients and carers requiring intervention from other professionals and liaising as appropriate.

      • To provide advice on symptom control/care management to other health care professionals, domiciliary agencies ,and care homes as appropriate.

      • To support the clinical team to undertake appropriate verification when a death has occurred, notifying relatives and carers as appropriate following TRF policy.

      • Provide high quality timely care for the deceased patient by administering last offices as appropriate and ensuring the safe and timely removal of the body from the home working within the procedures in place within the community.

      • To review, promote and comply with TRF Policies, Procedures & Guidelines PPG To ensure PPGs are regularly reviewed and clearly demonstrate up to date evidenced based practices

      • To understand the obligation to comply with external stakeholder policies.

      • To be responsible for the safety and positive experience of student nurses and others undertaking placements in TRF

      • To assist the Registered Manager in compiling standards to promote a high quality of patient care.

      • To administer drugs and assist with their ordering and storage in the home environment, in accordance with the NMC Standards for the Administration of Medicines. To recognise and report any errors or omissions in the administration of medications.

      • To work to develop quality improvement initiatives with the Senior Leadership Team

      • In conjunction with the Registered manager to support all volunteers and ensure appropriate governance systems are in place

      • Support data collection and provide clinical data reports for the Board of Trustees

      • Oversee the day-to-day management of Systmone, ensuring staff are competent and confident in documenting patient care and interventions

      • Recognise, Respond and Report any safeguarding concerns and escalate accordingly.

      • In conjunction with the Registered Manager report and manage any complaints or safeguarding concerns from patients, their relatives, or others acting on their behalf and assist with investigation of complaints as necessary

      • To support the Fundraising& Operational manager and support the clinical team to contribute Fundraising Events

      • To contribute to the Health & Safety Committee

      • To adhere to the NMC Code of Professional Conduct.

      • To offer a high level of emotional support to patients and/or carers particularly during the breaking of bad news and around time of death.

      • To contribute to bereavement care offered to families by facilitating bereavement support visits and assessing risk factors for bereavement, referring to TRF bereavement and counselling service, as necessary.

      • To sensitively handle out of hours enquires from patients and carers.

      • To develop and maintain effective working relationships with the Senior Leadership Team, Trustees, Fundraising Team, Administrative staff ,and volunteers.

      • To promote the aims and ideals of The Rosemary Foundation to the public.
